Porter Adventist Hospital wouuld like you to complete the Cardiac Risk Quiz and a registered nurse from Porter's Ask-A-Nurse Program will contact you by phone within three to five business days to discuss your results if they indicate a potential risk for developing heart disease. Wish they did this where I live!
